Regional Partnership Board and Strategic Leadership Group Meetings: November 2024

The Regional Partnership Board consists of representatives from across the third, housing and independent sectors along with independent members, health and local authority colleagues. They meet every two months to review progress on each of our Joint Area Plan commitments and to consider how we can amplify our impact by working together. The Strategic Leadership Group acts as the engine room for the RPB ensuring that its plans are driven forward at pace. An overview of our recent discussions is provided below:

@Home Highlights

The @Home programme is one of the RPB’s most significant programmes, working with partners to deliver an integrated community care system across the region. Current developments include:

  • Enhanced Community Care Model: we’re developing a business plan to deliver a range of community-based services which will reduce the rate at which current demand is rising for long term care and hospital treatment.

  • Place Based Planning: we’re also working to bring together a shared understanding of resources which are currently in place within each of nine clusters across our region. By sharing our knowledge on physical infrastructure, transport links, third sector provision, deprivation, etc. we hope to build a better understanding of our local places and opportunities for future development.

  • Safe@home: The work builds on the recent development of the Safe@home service which already showing a positive impact for people who might otherwise require an emergency hospital admission. Did you know the Safe@home team is now supporting 23 people per week on average, and reducing a similar number of hospital admissions as a result.

Winter Planning

The RPB team is working with operational colleagues to support winter planning in the following ways;

  • An action plan to reduce Package of Care Delays across the region.
  • a regional response to the 50 day integrated care winter challenge, including a 10 best practice recommendations from across Wales which you can view here.

Funding and Performance

Our thanks to everyone who contributed to our Quarter 2 reports which have been positively received by Welsh Government.

  • You can view dashboards demonstrating progress against our Area Plan Delivery Plan and for each funded project here.
  • In general, all workstreams are progressing to plan despite a few delays in our Unpaid Carer and Learning Disability programmes whilst recruitment and new contracts are underway.
  • The proposed budget for 2025-26 has now been agreed by SLG and RPB pending final confirmation by Welsh Government on all funding assumptions for the coming year. The team hope to communicate final allocations to all SROs before the end of January 2025.
  • For now, spend and performance should continue to be managed in line with the end of year forecasts submitted in Q2. As always, please let the RPB team know of any changes as soon as you can.
