People with Learning Disabilities
We want every person with a learning disability to live as independently as possible in their local community………
We work with the people and organisations who provide services for people with a learning disability, whether this be in health, local authorities, third sector or communities, to explore where working together can help improve the experiences of people, better meet needs and improve quality of life.
We want people to be able to live the lives they want to lead; living locally where they “feel good and well”, where they feel valued and included in their communities and have equal access to appropriate support that ensures independence, choice and control.
It is very important that we work with people with a learning disability, their families, carers to make sure any changes are informed by the real and lived experiences of people.

We have a delivery plan for every year that sets out what we want to achieve, our priorities for this year are:
- Better understand the local population of people with a learning disability so we can make sure the right services are available
- Work better with people to make sure that their lives experiences inform what happens, for example through supporting a new engagement network called Shine Together
- Improve the model of health care for people in our region
- Help prevent things from getting worse, for example through making it easier for everyone with a learning disability to receive an annual health check from their GP
- Make the buildings and places that provide services better
- Help make the change from childhood to adulthood easier, a period called transition

Support Planning
Support Planning, delivered through RPB partnership working, helps people with learning disabilities to live more independently through person-centred, co-produced support. This approach builds confidence, strengthens daily living skills and improves wellbeing, enabling people to have greater choice and control over their lives.
Transition Support
Transition support, delivered through RPB partnership working, helps young people with learning disabilities move into adulthood with the right support in place. Through coordinated, person-centred planning, young people and families feel more prepared and confident, leading to smoother transitions and more positive long-term outcomes.
