Adolescent Resources Centre

Adolescent Resource Centres (ARC) provide therapeutic support for young people on the edge of care, delivered through partnership working across the Regional Partnership Board. This trauma-informed, relationship-based approach helps young people feel heard, build trust and improve emotional wellbeing, with families reporting greater understanding, confidence and more positive futures.

Transition Support

Transition support, delivered through RPB partnership working, helps young people with learning disabilities move into adulthood with the right support in place. Through coordinated, person-centred planning, young people and families feel more prepared and confident, leading to smoother transitions and more positive long-term outcomes.

Support Planning (Learning Disabilities)

Support Planning, delivered through RPB partnership working, helps people with learning disabilities to live more independently through person-centred, co-produced support. This approach builds confidence, strengthens daily living skills and improves wellbeing, enabling people to have greater choice and control over their lives.

Young Carers

Young Carers support, delivered through our Starting Well and Living Well Programmes, helps children and young people balance caring responsibilities with their own wellbeing, education and social lives. Through early identification, tailored support and access to breaks and activities, young carers feel more supported, confident and able to thrive.

MDT / Accelerated Clusters (@Home / ICCS)

MDT and Accelerated Cluster working, delivered through RPB partnership working, brings health, social care and third sector teams together to provide coordinated, person-centred care closer to home. This integrated approach improves outcomes, reduces hospital admissions and ensures people receive the right support at the right time.

Mental Heath Matters/Dementia/ Opening Doors

Mental Health Matters support, delivered through RPB Ageing Well Programme, promotes wellbeing and social connection through community-based initiatives such as Opening Doors to Brain Workout. This approach helps people stay active, connected and supported, improving mental wellbeing and reducing isolation.

Capital Programme

The Cardiff and Vale Regional Partnership Board (RPB) Capital Programme for 2025–26 outlines a total investment of £8.13 million, with the majority allocated to Health and Care Fund (HCF) Objective 1 (£5.42 million), alongside targeted funding for Objectives 2 and 3 and the Intermediate Care Fund (IRCF). Investment is distributed across key priority groups, with equal emphasis (28% each) on people with learning disabilities and neurodivergence (LD/ND), older people, and children and young people (CYP), alongside support for unpaid carers (9%) and emotional health and wellbeing (6%). Spend across partners is led by Cardiff Council (51%), followed by the third sector (35%), with smaller contributions from the Vale of Glamorgan Council and Cardiff and Vale UHB (7% each). This programme is delivering tangible outcomes, including 24 capital schemes, 129 additional bed spaces, and 34 new placements to support both children with complex needs and adults to maintain independence. Overall, the investment is enabling more than 2,185 citizens to remain independent at home or within their communities, reinforcing the RPB’s focus on prevention, community-based care, and integrated service delivery.
