Strengthening integrated community care, supporting prevention and planning for the future were key priorities at September’s meetings of the Cardiff and Vale Regional Partnership Board (RPB) and Strategic Leadership Group (SLG).
Integrated Community Care in Action
The RPB’s September meeting featured a deep dive into the Integrated Community Care System (ICCS), showcasing how partners are working together to deliver more joined-up, preventative and community-focused care.

Three examples highlighted this approach:
- Llys y Goetre: Supporting older people to remain independent through housing with care, integrated services and community connections.
- Thompson Street and Barry Campus: Bringing health, social care, housing, wellbeing and community services together to improve access to local support.
- Integrated Neighbourhood Teams: Demonstrating how multidisciplinary working can help prevent falls, reduce social isolation and support independence.
Members reinforced the importance of stronger partnerships, meaningful community involvement and sharing good practice to improve outcomes across Cardiff and the Vale.
Planning for the Future of Regional Investment
The SLG welcomed confirmation that the Regional Integration Fund (RIF) would continue on a rolling five-year basis, providing greater confidence for future planning, although detailed funding allocations remain to be confirmed.
Discussions focused on protecting effective services, strengthening prevention and early intervention, and ensuring future investment delivers meaningful outcomes for local communities.
Supporting Communities and Winter Wellbeing
The SLG also considered opportunities to reinvest available funding into regional priorities, including dementia memory assessments, support for children and young people with complex needs, neurodevelopmental support and the First 1,000 Days programme.
Particular emphasis was placed on strengthening third-sector involvement in winter planning, helping people remain well at home, avoiding unnecessary hospital admissions and supporting safe discharge from hospital.
Strengthening Partnership Working
Both meetings highlighted the importance of collaboration, transparency and shared responsibility across health, local authorities, housing and the third sector.
The RPB also agreed to introduce an annual Compassionate Leadership self-assessment across its partnership arrangements, supporting continued learning and improvement.
Together, these discussions reflect a shared commitment to delivering more integrated, preventative and person-centred health and care services for communities across Cardiff and the Vale of Glamorgan.
