Safe at Home Service – A year on

Safe at Home is a Cardiff and Vale UHB Service that launched in January 2024. The service aims to assess, treat and care for patients safely in their place of residence, preventing WAST conveyances and  hospital admissions. We also look to support secondary care by facilitating early discharges in collaboration with Local Authority and existing community services such as Community Nursing and CRT. We are an intermediate, multidisciplinary service consisting of geriatric, medical, nursing and support worker professionals. Referrals are received from both the community (GPs, community nurses, CRT) and from the EU and secondary care. S@H work collaboratively with the Welsh Ambulance Service, actively triaging and pulling patients from the WAST stack with the aim of stepping down an ambulance.  

Safe at Home has supported more than 900 patients since the launch, with a rate of 80% of patients being kept at home. The service is continuing to develop and is heading into phase two, which will consist of the team delivering IVs in the community. S@H is currently working with emergency and acute medicine to establish daily virtual board rounds to increase the rate of patients being discharged earlier.
