UK Dementia Conference – #UKDC24

The RPB is very proud of Versha Sood, Improvement and Development Office for Dementia who presented at #UKDC2024 on how to actively and meaningfully engage with people in their own communities, through co-production and listening to those with lived experience. Continuing to break stigma and taboos, and help people access Dementia related service and build long-lasting relationships which led to co-production within Cardiff and Vale’s Dementia Programme.
Safe@Home – One Year On
Safe at Home is a Cardiff and Vale UHB Service that launched in January 2024. The service aims to assess, treat and care for patients safely in their place of residence, preventing WAST conveyances and hospital admissions.

We also look to support secondary care by facilitating early discharges in collaboration with Local Authority and existing community services such as Community Nursing and CRT. We are an intermediate, multidisciplinary service consisting of geriatric, medical, nursing and support worker professionals. Referrals are received from both the community (GPs, community nurses, CRT) and from the EU and secondary care. S@H work collaboratively with the Welsh Ambulance Service, actively triaging and pulling patients from the WAST stack with the aim of stepping down an ambulance.
Safe at Home has supported more than 900 patients since the launch, with a rate of 80% of patients being kept at home. The service is continuing to develop and is heading into phase two, which will consist of the team delivering IVs in the community. S@H is currently working with emergency and acute medicine to establish daily virtual board rounds to increase the rate of patients being discharged earlier.

Emergency Continuing Care Workshop



On 22nd January, partners from across health and social care came together for a workshop at County Hall focussed on how we might develop some of the brilliant work in the community which Is helping keep people well and avoid the need to go to hospital.
Services such as Safe@home discussed ways of linking more closely to other areas to help more people keep well.

Adref – Opening Ceremony
On Friday 24th January, YMCA England & Wales opened their ‘Adref’ building with a wonderful opening ceremony, attended by members of the Cardiff and Vale Regional Partnership Board, Jenny Rathbone MS and Jo Stevens MP, Secretary of State as well as YMCA CEO Andrew Templeton.

‘Adref’ is an exemplary project with an industry leading ‘green’ design helping to meet the desperate need for more affordable social housing. Adref supports young people at risk of homelessness marking a new chapter for YMCA Cardiff and for the young people who will now have a safe and supportive place to call home. Each unit has its own air source heat pump, and the new site will soon house 18 young people, with 11 moving in as soon as a week after the launch.
Jointly Funded Extra Care Housing Scheme

The Vale of Glamorgan Council and Wales & West Housing (WWH) have begun work on an extra care housing scheme in Penarth for more than 70 people over 55. It’s great to see the development of this project with significant investment from the Regional Partnership Board’s capital funding sources.The development includes self-contained apartments, with tailored on-site care and support, for people who want to live independently in their own homes within a supportive community.
Part funded by the Housing with Care Fund and a Social Housing Grant from Welsh Government, the £20 million project will see 70 affordable apartments constructed by WWH after the Council transferred land to it on a long-term lease. That is the latest step in a larger project to integrate new and existing older persons’ homes with social care and health facilities at the location.
