The Welsh Government launched a new three-year Learning Disabilities Transformation Strategy in September which aims to improve community support, reduce hospital admissions and lay the groundwork for a 10-year strategy.
The programme will focus on four core areas including Data, information and advice: Improving how information is shared and accessed by individuals and families.
Models of integrated community care: Building joined-up services across health, social care, and prevention, Specialist services: Enhancing crisis support and managing inpatient care and Accommodation and housing solutions: Expanding local, independent living options.
We held a joint screening of the launch through the Learning Disabilities Partnership Group, and we look forward to seeing progress.
