Cardiff and Vale Regional Partnership Board is proud to share that the Safe@Home team has been shortlisted for a Welsh Government award in recognition of their outstanding work in helping to keep people out of hospital.
The team has been invited to London on Monday 31st March to celebrate this achievement and showcase the impact of their innovative approach to admission avoidance and early discharge.
Since going live, Safe at Home has supported individuals by:
- Preventing avoidable hospital admissions
- Accepting timely referrals from secondary care
- Saving vital hospital bed days
