Rydym yn gwybod pa mor anodd y gall fod i ddod o hyd i help pan fo pobl ei angen fwyaf a bod yr heriau y mae gofalwyr di-dâl yn eu hwynebu wedi dod yn anoddach fyth dros y flwyddyn ddiwethaf.

Mae gan y Ddeddf ddiffiniad newydd o ofalwr fel person sy’n darparu neu’n bwriadu darparu gofal ar gyfer oedolyn neu blentyn anabl.  Mae hyn yn dileu’r gofyniad bod yn rhaid i ofalwyr fod yn darparu ‘swm sylweddol o ofal yn rheolaidd’.  

Rhaid i awdurdodau lleol gynnig asesiad gofalwr i unrhyw ofalwr os yw’n ymddangos i’r cyngor y gallai fod gan y gofalwr anghenion am gymorth.  

Porth Gofalwyr Caerdydd a’r Fro 

PMae’r Gronfa Gofal Integredig wedi’i defnyddio i lansio’r Porth Gofalwyr ar gyfer Caerdydd a Bro Morgannwg. Dan arweiniad Ymddiriedolaeth Gofalwyr De-ddwyrain Cymru, mae’r tîm yn darparu gwybodaeth a chymorth i ofalwyr di-dâl, gan eu helpu i wneud y gorau o’u bywyd ochr yn ochr â’u rôl gofalu a chynnal eu hannibyniaeth.  

Mae’r tîm yn helpu gofalwyr gyda phethau fel: 

  • Deall pa gymorth sydd ar gael i ofalwyr 
  • Cyfeirio gofalwyr at wasanaethau lleol a’u helpu i’w defnyddio

  • Cynnig cyfleoedd hyfforddi a datblygu i ofalwyr  

Prosiect ‘Amser i Mi’ Gofalwyr Ifanc 

Mae gofalwyr ifanc yn aml iawn yn cymryd cyfrifoldebau pobl mewn oed. Mae hyn yn golygu eu bod yn colli allan ar y pethau y gall plentyn arall eu cymryd yn ganiataol – y cyfle i ddysgu, chwarae a chael hwyl. Daeth eu rôl hyd yn oed yn anoddach yn ystod cyfnod clo COVID 19.  

Yn ystod 2020 – 21 

  • Cynhaliwyd 443 o sesiynau cymorth unigol un i un 
  • Cyflwynwyd 262 o weithgareddau rhithwir 
  • Cyflwynwyd 132 o sesiynau cymorth i’r teulu 
  • Daeth 75 o ofalwyr ifanc newydd ymlaen i ofyn am gymorth 
  • 30 o sesiynau seibiant grŵp 30 o sesiynau seibiant grŵp

Stori Millie  

Mae Gweithiwr Lles Gofalwyr o Borth y Gofalwyr wedi bod yn cefnogi Millie, menyw 22 oed sy’n byw yng Nghaerdydd.  

Mae Millie yn gofalu am ei phartner sydd â phroblemau iechyd meddwl gan gynnwys Mae Millie yn gofalu am ei phartner sydd â phroblemau iechyd meddwl gan gynnwys awtistiaeth, iselder, mae’n ddeubegynol ac weithiau mae wedi hunan-niweidio ac wedi ceisio lladd ei hun. 

Cysylltodd Millie â’r Porth Gofalwyr i ddechrau i ofyn am grant o gronfeydd Llywodraeth Cymru ar gyfer microdon a dyfarnwyd arian.  Mae prynu’r meicrodon yn golygu ei bod bellach yn gallu paratoi prydau sy’n ei rhyddhau i dreulio mwy o amser yn gofalu am anghenion ei phartner sy’n rhai beichus a dwys. 

Mae partner Millie yn cael newid eithafol o ran hwyliau, wedi’u gwaethygu gan y cyfnod clo. Bu’n rhaid iddi roi sylw iddo gyda phethau fel ei helpu i dreulio gormod o egni gyda theithiau cerdded yn y parc am 3 am, ymweliadau ag adrannau damweiniau ac achosion brys yn dilyn ymdrechion i gyflawni hunanladdiad a hwyliau isel iawn.  Mae hyn wedi gadael Millie wedi blino’n gorfforol ac yn emosiynol.

Mae’r Gweithiwr Lles Gofalwyr wedi darparu clust i wrando ac wedi trefnu 6 sesiwn cwnsela drwy gyllid Llywodraeth Cymru, yn ogystal â gweithdai lles i ddelio â straen a gorbryder a thechnegau rheoli cysgu. 

Dywedodd Millie: “Diolch am eich help.  Dydw i ddim yn gofyn fel arfer ond bydd hyn yn fy helpu”.  

Stori Grace  

Ar argymhelliad ffrind, darganfu Grace wefan yr Ymddiriedolaeth Gofalwyr De Ddwyrain Cymru ar ôl chwiliad “google”. Mae Grace yn rhiant-ofalwr i’w mab sydd wedi cael diagnosis o awtistiaeth.  Mae mab Grace yn amharod i gael ei adnabod fel un awtistig gan ei fod yn teimlo bod stigma o’i gwmpas ac mae wedi cael ei fwlio, gan gynnwys cael beic wedi’i ddwyn.  Mae hyn wedi ei arwain i wrthod ystod o gymorth gan gynnwys cwnsela.  Yn ogystal, mae Grace yn credu bod ei phartner, sy’n eithaf ifanc, wedi bod yn “hongian o gwmpas gyda’r dorf anghywir”  

Roedd Gweithiwr Lles Gofalwyr o Borth y Gofalwyr wedi cyfeirio Grace at y Swyddog Cymorth Cymunedol yr Heddlu lleol i edrych i mewn i’r lladrad beiciau, darparu gwybodaeth am wasanaethau cymorth ar gyfer “plant cythryblus” a allai helpu’r mab a’r partner, gwybodaeth am dai, pe bai’r teulu’n dymuno symud i ardal is-radd a bod Grace wedi’i threfnu ar gwrs lles ar-lein o’r enw “Stress & Anxiety: Coping Techniques” y gall yr Ymddiriedolaeth Gofalwyr De Ddwyrain Cymru eu darparu oherwydd cyllid Llywodraeth Cymru. Yn olaf, cafodd Grace ei chyfeirio at Lythyr Mynegai’r Gwasanaeth Gwybodaeth i Deuluoedd.  

Dywedodd Grace fod y gweithdai lles yn “gefnogol iawn” ac oni bai am y chwiliad “google”, byddai wedi aros yn “ofalwr cudd”.  

Stori Irene  

Mae Irene yn wraig 90 oed a gafodd ei chyfeirio at yr Ymddiriedolaeth Gofalwyr De Ddwyrain Cymru gan y Gwasanaethau Cymdeithasol. Mae’n ofalwr i’w gŵr 92 oed y mae wedi bod yn briod ag ef ers 70 mlynedd ac mae bellach ganddo ddementia. Mae’n cadw ei gŵr mor brysur ac wedi’i symbylu â phosibl drwy ddarllen papurau dyddiol a mynd am dro i fyny ac i lawr eu dreif.  

Mae Irene yn disgrifio ei gŵr fel ei “byd cyfan”.  

Mae Irene wedi bod yn poeni’n ddiweddar am y gofal i’w gŵr pe bai ei hiechyd ei hun yn dechrau dirywio.  Mae meddyliau o’r fath wedi mynd yn fwy difrifol gan ei bod i fod i gael llawdriniaeth fawr yn yr ysbyty.  

Ffoniodd Irene Weithiwr Lles Gofalwyr ar Borth y Gofalwyr a thrafododd ei phryderon yn faith a siaradodd am y ffaith y gallai fod angen i’r gŵr fynd i ofal awdurdod lleol.  Trafododd y Gweithiwr Lles hefyd y posibilrwydd o wneud cais am becyn gofal i gynnal annibyniaeth cyhyd ag y bo modd.  


Roedd Irene yn gwerthfawrogi’n fawr yr amser a’r lle i rannu ei meddyliau a’i phryderon mewn lleoliad cyfrinachol a dywedodd:

Unpaid Carers

The Social Services and Well-being (Wales) Act 2014

The Act has a new definition of a carer as a person who provides or intends to provide care for an adult or disabled child. This removes the requirement that carers must be providing ‘a substantial amount of care on a regular basis’.
Local authorities must offer a carer’s assessment to any carer where it appears to the council that the carer may have needs for support.

Cardiff & Vale Carers Gateway

The Integrated Care Fund has been used to launch the Carers Gateway for Cardiff and the Vale of Glamorgan. Led by The Carers Trust Southeast Wales, the team provide information and support to unpaid carers, helping them to make the most of their life alongside their caring role and maintain their independence.

The team helps carers with things like:

  • Understanding what support is available for carers
  • Signposting and supporting carers to access local services
  • Identifying new services that are needed to help carers
  • Raising awareness on the issues carers face
  • Providing training and development opportunities for carers

Outcomes

Young carers often take on grown-up responsibilities. This means that they miss out on the things that other child may take for granted – the chance to learn, play and have fun. Their role became even harder during COVID 19 lockdown.

People with Learning Disabilities

Our vision is that people with learning disabilities have a good quality of life and are supported to live the lives they want to lead; living locally where they “feel good and well”, where they are valued and included in their communities and have equal access to appropriate support that ensures independence, choice and control.

As a Partnership, we have worked together with people with a learning disability, their families, carers and the third and independent sector to produce a clear direction for the planning and delivery of adult learning disability services across the region.

This strategy describes our shared vision, the core principles that we believe are fundamental to service design and delivery, and the key issues that need to be addressed in the forthcoming years in order to deliver effective, safe and good quality services. This includes ensuring there is sufficient capacity and an appropriate range of provision to meet local need.

PURPOSE – WHY HAVE A STRATEGY?

The purpose of our strategy is to describe how Cardiff and Vale of Glamorgan Councils and Cardiff and Vale University Health Board will work together to respond to the needs and aspirations of our adults with a learning disability within the context of national legislation and guidance, wider local plans and available resources.

At a local level, our strategy will inform what services each of the three organisations will individually or jointly commission over the next five years.

The Impact For Carers

Millie’s story

A Carers Wellbeing Worker from the Carers Gateway has been supporting Millie, a 22-year-old woman who lives in Cardiff.
Millie cares for her partner who has mental health issues including autism, depression, bi-polar and on occasions has both self-harmed
and attempted suicide.

Millie initially contacted the Carers Gateway to request an emergency grant from Welsh Government funds for a microwave and an award was made. The purchase of the microwave means that she can now batch-prepare meals which frees her to spend more time tending to her partners needs which are demanding and intense.

Millie’s partner is currently having extreme mood changes, exacerbated by the lockdown. She has had to attend to him with such things as helping him spend excess energy with walks in the park at 3 am, visits to A&E following suicide attempts and deep depressive moods. This has left Millie exhausted physically and emotionally.

The Carers Wellbeing Worker has provided a listening ear and has arranged for 6 counselling sessions through the Welsh Government funding, as well as wellbeing workshops to deal with both stress & anxiety and sleep management techniques.

Millie said:

“Thanks for your help. I don’t normally ask but this will help me”.

Grace’s story

At the recommendation of a friend, Grace discovered the CTSEW website after a “google” search. Grace is a parent carer for her son who has been diagnosed with autism. Grace’s son is averse to being identified as autistic as he feels there is a stigma around this and he has experienced bullying, including having a bicycle stolen. This has led him to refuse a range of support including counselling. Additionally, Grace believes that her partner, who is quite young, has been “hanging around with the wrong crowd”

A Carers Wellbeing Worker from the Carers Gateway signposted Grace to the local PCSO to look into the bicycle theft, provided information about support services for “troubled kids” which might help both the son and the partner, information about housing, should the family wish to move to a lowercrime area and Grace was booked onto an on-line wellbeing course entitled Stress & Anxiety: Coping Techniques which CTSEW are able to provide due to Welsh Government funding. Lastly, Grace was directed to the Family Information Service’s Index Letter.

Grace reported that the wellbeing workshops were “really supportive” and had it not been for the “google” search, she would have remained a “hidden carer”.

Irene is a 90-year-old lady who was referred to CTSEW by Social Services. She is a carer for her 92-year-old husband to whom she has been married for 70 years and who now has dementia. She keeps her husband as busy and stimulated as possible by reading daily newspapers and taking walks up and down their driveway.
Irene describes her husband as her “whole world”.

Irene has been having concerns lately over the care for her husband should her own health begin to deteriorate. Such thoughts have become more acute as she is due to have a major operation in hospital.

Irene phoned a Carers Wellbeing Worker on the Carers Gateway and discussed her concerns at length and talked about the fact that there may be a point at which the husband may need to go into local authority care. The Wellbeing Worker also discussed the possibility of applying for a care package to maintain independence for as long as possible.

Irene really appreciated the time and space to share her thoughts and concerns in a confidential setting and said:

“thank you for listening to me”
